We power our work with Outreach Grid—a cutting-edge, all-in-one platform that connects every part of the homelessness response system. From outreach and dispatch to real-time Navigation Center bed-management, encampment management, case tracking, shelter placement and HMIS integration, this platform allows us to collaborate in real time and collect data right where it matters most: in the field. With every interaction, we’re able to build a complete and up-to-date profile of each individual we serve, helping case managers make faster, smarter referrals and chart a clearer path toward permanent housing.

The Approach

Meeting People Where They Are - Literally

Homelessness doesn’t happen in an office, and neither does our response. That’s why our HOPE Center Mobile Office is out on the streets every day, bringing essential services directly to those who need them most. These mobile units offer not just a place to connect, but a safe, welcoming space for stabilization, respite, and real-time access to shelter, case management, and transportation. It's about being there—wherever there is.

At the heart of everything we do is a dedicated team of seasoned outreach professionals. With compassion and commitment, they build trust with our unhoused neighbors and work hand-in-hand with county agencies, nonprofit partners, and local law enforcement. Together, we serve the cities of Brea, Buena Park, Cypress, Fullerton, La Habra, La Palma, Orange, Placentia, Stanton, and Yorba Linda—making sure no one slips through the cracks.

The HOPE Center is more than a service provider—it’s a command center for change. We combine evidence-based practices with real-time data to tackle homelessness head-on.
